Scope of This Policy
This Privacy Policy explains how Dallas Spine Center (“we,” “us,” or “our”) collects, uses, and shares information from visitors to www.dspinecenter.com and any pages or forms we operate on it. It applies to information collected through this website and to related communications such as appointment requests, emails, and text messages.
This website policy is not our HIPAA Notice of Privacy Practices. Protected health information created or maintained as part of your care as a patient is governed separately by the Health Insurance Portability and Accountability Act (HIPAA) and by our Notice of Privacy Practices, which is provided to patients at the office and available on request.
Information you submit through this website, such as a name and phone number on an appointment request, becomes part of your patient record once you become a patient, and from that point it is handled under HIPAA and our Notice of Privacy Practices rather than under this policy.
Information We Collect
Information you give us
You do not have to give us any personal information to browse this website. You choose to provide information when you contact us or request care, and that may include:
- Your name, phone number, and email address
- The reason for your visit or a general description of your symptoms or goals
- Preferred appointment times and scheduling details
- Insurance or payment questions you choose to send us
- Anything else you include in a message, form, review, or reply
Please do not send detailed medical information through this website. Website forms and ordinary email are not secure or encrypted channels. Send only what is needed to reach you and schedule a visit, and share clinical details with us by phone or in person instead.
Information collected automatically
Like most websites, ours automatically collects limited technical information when you visit, which may include:
- IP address and general location inferred from it, such as city or region
- Browser type, device type, operating system, and screen size
- Pages viewed, links clicked, time spent, and the site or search that referred you
- Date and time of your visit and similar server log data
Information from third parties
We may receive information from services we use to run the practice, such as scheduling or review platforms, insurance and financing partners when you apply through them, and analytics providers. We do not purchase personal information about you from data brokers.
Cookies and Tracking Technologies
This website uses cookies and similar technologies, which are small files or scripts that let the site function properly and help us understand how it is used. These fall into a few categories:
- Essential: required for the site to load, keep pages secure, and remember basic preferences such as accessibility settings.
- Analytics: used to understand which pages people read and where visitors have difficulty, so we can improve the site.
- Advertising and social: where used, these help measure whether our advertising reaches people who are looking for care, and may be set by platforms such as search or social media services.
Most browsers let you refuse or delete cookies through their settings. Blocking some cookies may cause parts of this website to stop working correctly. This site does not currently respond to browser “Do Not Track” signals, because no common standard for honoring them has been adopted.
Health-related browsing
We are mindful that the pages you read on this site can suggest a health concern. We do not knowingly use website analytics or advertising tools in a way that identifies you personally and links you to a specific health condition, and we do not sell information about your browsing.
Third-Party Services Used on This Site
Some features of this site are provided by outside companies. When you use those features, the provider may collect information under its own privacy policy, which we do not control. These currently include:
- Google Maps for our location map and directions
- Cherry for patient financing information and applications, on our Payment Plans page
- Review platforms that display patient reviews on our site
- Accessibility tools that provide the accessibility menu
- Analytics providers that measure site usage
- Our website host and email provider, which process data on our behalf
If you apply for financing, that application is submitted to and decided by the financing company, not by us. We do not receive or store your full financial application, credit report, or account numbers from that process.
How We Use Information
We use the information described above to:
- Respond to your questions and requests
- Schedule, confirm, reschedule, and remind you about appointments
- Verify insurance benefits and answer questions about cost and payment
- Provide, coordinate, and improve your care once you become a patient
- Operate, secure, and improve this website
- Send practice updates or offers if you have asked to receive them
- Meet legal, regulatory, licensing, and recordkeeping obligations
- Establish, exercise, or defend legal claims where necessary
Text Messages, Calls, and Email
If you give us your phone number, you agree that we may contact you about your care, appointments, and account by phone call, voicemail, and text message, including through automated systems where applicable.
- Message and data rates may apply, and message frequency varies.
- You can stop appointment or marketing texts at any time by replying STOP, and get help by replying HELP.
- You may opt out of marketing emails using the unsubscribe link in any such email.
- Opting out of marketing messages does not stop necessary communications about scheduled appointments, treatment, or billing.
- Consent to receive marketing messages is not a condition of receiving care.
Your Privacy Choices and Rights
Regardless of where you live, you may contact us to ask what information we hold about you, to correct it, to request deletion of website-related information, or to opt out of marketing. We will respond as required by applicable law.
Colorado residents
The Colorado Privacy Act gives Colorado consumers rights to access, correct, delete, and obtain a portable copy of certain personal data, and to opt out of targeted advertising, the sale of personal data, and certain profiling. That law contains an exemption for information handled under HIPAA, and it applies only to businesses meeting specific size thresholds that a practice of our size generally does not meet. We nonetheless honor these requests as a matter of policy. To make a request, use the contact details below, and we will verify your identity before responding.
Other states
Residents of other states with comprehensive privacy laws, including California, Virginia, Connecticut, and Utah, may have similar rights. If you are covered by one of those laws, contact us and we will handle your request in accordance with it. We will not discriminate against you for exercising any privacy right.
Your rights as a patient
Rights concerning your medical record, including access, amendment, an accounting of disclosures, and restrictions on use, are governed by HIPAA and described in our Notice of Privacy Practices. Ask the front desk for a copy.
How We Protect Information
We use administrative, technical, and physical safeguards intended to protect the information we hold, including secure hosting, access controls, and staff training on privacy obligations. Our website is served over an encrypted connection.
No website, email system, or method of electronic storage is completely secure, and we cannot guarantee absolute security. Information you send through unencrypted channels such as ordinary email or a web form travels at your own risk, which is why we ask you not to send detailed medical information that way.
How Long We Keep Information
We keep website inquiries and communications for as long as needed to respond to you and for our ordinary business records. Patient records are retained for the period required by Colorado law and professional standards, which is longer than the retention of general website data, and we may retain information as needed to comply with legal obligations, resolve disputes, or enforce our agreements.
Children’s Privacy
This website is intended for a general adult audience and is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. Parents and guardians schedule and consent to care for minor patients, and information about a minor patient’s care is handled under HIPAA and Colorado law. If you believe a child has provided information through this website, contact us and we will delete it.
Links to Other Websites
Our site links to websites we do not operate, including insurers, financing companies, review platforms, and informational resources. We are not responsible for the content, security, or privacy practices of those sites, and this policy does not apply to them. Review the privacy policy of any website you visit.
Changes to This Policy
We may update this Privacy Policy to reflect changes in our practices, technology, or the law. When we do, we will revise the “last updated” date at the top of this page, and material changes will be indicated clearly. Your continued use of this website after an update means you accept the revised policy.
