Scope of This Policy
This Privacy Policy explains how Dallas Spine Center LLC (“we,” “us,” or “our”) collects, uses, and handles information from visitors to www.dspinecenter.com and any pages or forms we operate on it. It applies to information collected through this website and to related communications such as appointment requests, emails, and text messages.
This website policy is not our HIPAA Notice of Privacy Practices. Protected health information created or maintained in connection with your care is handled under HIPAA, other applicable law, and our Notice of Privacy Practices. You may request that notice from our office.
Our appointment request form is intended for contact details and general scheduling requests. Please do not submit medical records, detailed symptoms, insurance identifiers, or other sensitive health information through this form or ordinary email.
Information We Collect
Information you give us
You do not have to give us any personal information to browse this website. You choose to provide information when you contact us or request care, and that may include:
- Your name, phone number, and email address
- General appointment requests and contact preferences
- Preferred appointment times and scheduling details
- General questions about scheduling, office hours, or fees
- Your SMS consent choices and records of your opt-in and opt-out requests
- Other non-sensitive information you choose to include in an inquiry
Please do not send detailed medical information through this website. Send only what is needed to reach you and schedule a visit. Please contact the office to arrange an appropriate way to share clinical details.
Information collected automatically
Like most websites, ours automatically collects limited technical information when you visit, which may include:
- IP address and general location inferred from it, such as city or region
- Browser type, device type, operating system, and screen size
- Pages viewed, links clicked, time spent, and the site or search that referred you
- Date and time of your visit and similar server log data
Information from third parties
We may receive information from services we use to run the practice, such as scheduling or review platforms, insurance and financing partners when you apply through them, and analytics providers.
Cookies and Tracking Technologies
This website uses cookies and similar technologies, which are small files or scripts that let the site function properly and help us understand how it is used. These fall into a few categories:
- Essential: required for the site to load, keep pages secure, and remember basic preferences such as accessibility settings.
- Analytics: used to understand which pages people read and where visitors have difficulty, so we can improve the site.
- Advertising and social: where used, these help measure whether our advertising reaches people who are looking for care, and may be set by platforms such as search or social media services.
Most browsers let you refuse or delete cookies through their settings. Blocking some cookies may cause parts of this website to stop working correctly. This site does not currently respond to browser “Do Not Track” signals, because no common standard for honoring them has been adopted.
Health-related browsing
We are mindful that the pages you read on this site can suggest a health concern. We do not knowingly use website analytics or advertising tools in a way that identifies you personally and links you to a specific health condition, and we do not sell information about your browsing.
Third-Party Services Used on This Site
Some features of this site are provided by outside companies. When you use those features, the provider may collect information under its own privacy policy, which we do not control. These currently include:
- Google Maps for our location map and directions
- Cherry for patient financing information and applications, on our Payment Plans page
- Review platforms that display patient reviews on our site
- Accessibility tools that provide the accessibility menu
- Analytics providers that measure site usage
- Our website host and email provider, which process data on our behalf
- GoHighLevel / LeadConnector for appointment request forms and communication management on our behalf
If you apply for financing, that application is submitted to and decided by the financing company, not by us. We do not receive or store your full financial application, credit report, or account numbers from that process.
How We Use Information
We use the information described above to:
- Respond to your questions and requests
- Schedule, confirm, reschedule, and remind you about appointments
- Verify insurance benefits and answer questions about cost and payment
- Provide, coordinate, and improve your care once you become a patient
- Operate, secure, and improve this website
- Send appointment confirmations, appointment reminders, and appointment scheduling updates by text only when you have opted into these messages
- Send marketing texts about special offers, discounts, and service updates only when you have separately opted into marketing messages
- Maintain consent records, manage communication preferences, and honor opt-out requests
- Meet legal, regulatory, licensing, and recordkeeping obligations
- Establish, exercise, or defend legal claims where necessary
Text Messages, Calls, and Email
Your SMS consent choices
Providing your phone number or submitting an appointment request does not, by itself, authorize text messages. We collect SMS consent through separate, optional checkboxes on our appointment request form. These checkboxes are not selected by default. You may choose appointment-related texts, marketing texts, both, or neither.
SMS consent is not required to submit a form, request an appointment, or receive care. A phone number may be required so our team can contact you, while both SMS consent choices remain optional.
Messages you may receive
- Non-marketing texts: appointment confirmations, appointment reminders, and appointment scheduling updates, if you select the non-marketing consent checkbox.
- Marketing texts: special offers, discounts, and service updates from Dallas Spine Center LLC, if you separately select the marketing consent checkbox.
We use your consent choices to determine which types of text messages you may receive. Consent to one category does not authorize the other.
Message frequency, rates, and help
- Message frequency may vary based on your appointments, requests, and the message categories you selected.
- Message and data rates may apply to messages sent to or received from us. Contact your wireless provider with questions about your text or data plan.
- Text HELP for assistance, or call Dallas Spine Center LLC at (469) 986-9171.
How to stop text messages
Reply STOP to any message from our SMS program to unsubscribe. We will send one confirmation of your opt-out, then stop sending text messages from the program unless you provide new, explicit SMS consent. You can enroll again through the consent options on our appointment request form.
If you decline or withdraw SMS consent, you can still request appointments and receive care. Please contact the office to arrange communication by phone or another available method.
Calls and email
We may use the contact details you provide to respond to your inquiry or coordinate scheduling by phone or email. You may unsubscribe from marketing emails using the unsubscribe link in those emails.
For more information about our messaging program, please review our Terms of Service. Please do not send protected health information or urgent medical requests by text message.
Your Privacy Choices and Rights
You may contact us to ask what information we hold about you, request corrections, request deletion of website-related information, or update your communication preferences. We will respond as required by applicable law and may need to verify your identity before acting on a request.
Applicable privacy rights
Depending on where you live and the laws that apply, you may have rights to access, correct, delete, or obtain a copy of certain personal information, or to limit certain uses of it. Contact us using the details below to make a request. We will not discriminate against you for exercising an applicable privacy right.
Some records may need to be retained to meet legal obligations or document and honor your SMS opt-out request.
Your rights as a patient
Rights concerning your medical record, including access, amendment, an accounting of disclosures, and restrictions on use, are governed by HIPAA and other applicable law and described in our Notice of Privacy Practices. Ask the front desk for a copy.
How We Protect Information
We use administrative, technical, and physical safeguards intended to protect the information we hold, including secure hosting, access controls, and staff training on privacy obligations. Our website is served over an encrypted connection.
No website, email system, or method of electronic storage is completely secure, and we cannot guarantee absolute security. Please limit website forms and ordinary email to general inquiries and scheduling details. Contact our office to arrange an appropriate way to share medical records or other sensitive health information.
How Long We Keep Information
We keep website inquiries and communications for as long as needed to respond to you and for our ordinary business records. Patient records are retained as required by applicable federal and state law and professional requirements. We may retain information as needed to comply with legal obligations, resolve disputes, or enforce our agreements. SMS consent and opt-out records are retained as needed to document your choices and honor your communication preferences.
Children’s Privacy
This website is intended for a general adult audience and is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. Parents and guardians schedule and consent to care for minor patients, and information about a minor patient’s care is handled under HIPAA and other applicable law. If you believe a child has provided information through this website, contact us and we will delete it.
Links to Other Websites
Our site links to websites we do not operate, including insurers, financing companies, review platforms, and informational resources. We are not responsible for the content, security, or privacy practices of those sites, and this policy does not apply to them. Review the privacy policy of any website you visit.
Changes to This Policy
We may update this Privacy Policy to reflect changes in our practices, technology, or the law. When we do, we will revise the “last updated” date at the top of this page, and material changes will be indicated clearly. Your continued use of this website after an update means you accept the revised policy.
