Acceptance of These Terms
These Terms of Service govern your use of www.dspinecenter.com and any content, forms, or features on it, operated by Dallas Spine Center LLC (“we,” “us,” or “our”). By accessing or using this website, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the site.
These Terms apply to your use of the website. They do not replace or modify any consent, financial responsibility, arbitration, or treatment agreement you sign as a patient at our office, and in the event of a conflict, the documents you sign at the office control as to your care.
Medical Disclaimer and No Doctor-Patient Relationship
The content on this website is general health information for education only. It is not medical advice, and it is not a substitute for examination, diagnosis, or treatment by a qualified healthcare provider.
Reading this website, submitting a form, or communicating with us electronically does not create a doctor-patient relationship. That relationship begins only when you are seen and accepted as a patient in our office after an appropriate examination.
Never disregard professional medical advice, or delay seeking it, because of something you read here. Descriptions of conditions, treatments, timelines, and outcomes on this site are general and may not apply to you. Only an in-person examination can determine what is actually causing your symptoms and whether our care is appropriate for you.
Medical emergencies
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this website, email, or text messages to seek help for an emergency. Warning signs described on this site, such as loss of bowel or bladder control, sudden severe pain, numbness spreading into the face, slurred speech, chest pain, or shortness of breath, require immediate medical attention, not a chiropractic appointment.
No Guarantee of Results
Results from chiropractic care and related therapies vary from person to person. Factors including your age, overall health, the nature of your condition, how long it has existed, and your consistency with a recommended plan all affect outcomes.
- Nothing on this website is a promise, guarantee, or warranty of any particular result, recovery time, or level of improvement.
- Any timelines, visit counts, success rates, or examples described reflect general clinical experience and are not predictions about your case.
- Patient reviews and testimonials reflect individual experiences and are not typical of every patient or a guarantee of similar results.
- Not every condition is appropriate for chiropractic care, and we refer patients elsewhere when that is the right course.
Appointment Requests and Communications
Appointment forms and messages on this site are requests, not confirmed appointments. An appointment exists only once our office has confirmed it directly with you. We cannot guarantee any particular appointment time or that we will see a given request promptly.
- Do not use website forms, email, or text messages for urgent or emergency needs.
- Use the appointment request form for contact details and general scheduling only. Do not send detailed medical information, insurance identifiers, or sensitive personal data through this form or ordinary email.
- You are responsible for providing accurate contact information and scheduling details.
- We may use the contact details you provide to respond to your request by phone or email. Providing a phone number does not, by itself, authorize SMS messages.
SMS messages require your separate, affirmative consent as described below and in our Privacy Policy. Accepting these Terms does not automatically enroll you in text messaging.
SMS Messaging Terms
Dallas Spine Center LLC operates an optional SMS messaging program for appointment-related communications and marketing updates. These messages are sent only to people who have opted into the relevant message category.
Message types
- Non-marketing messages: appointment confirmations, appointment reminders, and appointment scheduling updates.
- Marketing messages: special offers, discounts, and service updates from Dallas Spine Center LLC.
Age Requirement (18+)
Participation in the Dallas Spine Center LLC SMS program is limited to adults aged 18 or older. By enrolling, you confirm that you meet this age requirement. A parent or legal guardian may subscribe using their own phone number to receive appointment messages concerning a minor in their care.
How you provide consent
You may opt in through the separate consent checkboxes on our appointment request form. The appointment-related and marketing consent options are independent, optional, and not checked by default. You may choose either category, both categories, or neither. Consent to one category does not authorize the other.
Consent to text messaging is not a condition of submitting a form, requesting an appointment, receiving care, or making a purchase. Providing your phone number or accepting these website Terms does not replace your separate SMS consent.
Message frequency and rates
Message frequency may vary based on your appointments, scheduling requests, and the message categories you selected. Message and data rates may apply to messages we send to you and messages you send to us. Contact your wireless provider with questions about your text or data plan.
How to unsubscribe
You can cancel the SMS service at any time by replying STOP to a text from our messaging program. We will send one message confirming that you have been unsubscribed. After that confirmation, you will receive no further text messages from the program unless you opt in again.
To re-enroll, return to our appointment request form and select the SMS consent options for the message categories you want to receive. We will resume text messaging only after obtaining your new, explicit consent.
Declining or withdrawing SMS consent does not affect your ability to request an appointment or receive care. Contact our office to arrange communication by phone or another available method.
Help and customer support
Reply HELP to a message from our program for assistance. You can also contact Dallas Spine Center LLC directly by calling (469) 986-9171.
Carrier liability
Carriers are not liable for delayed or undelivered messages.
Privacy and appropriate use
For information about how we handle your mobile information and SMS consent, please read our Privacy Policy. Do not send protected health information, medical records, or urgent medical requests through the SMS program.
New Patient Offers, Fees, and Insurance
Promotional pricing described on this site, including any new patient visit or first-session offers, applies to new patients, covers only the services described in the offer, and cannot be combined with other offers unless we state otherwise. Offers may be modified or discontinued at any time, and offers do not apply where prohibited by law or by the terms of a health plan, and may not be available in connection with federal healthcare program benefits.
- Fees for services beyond a promotional offer are quoted before care begins.
- Insurance information on this site is general. Coverage, benefits, deductibles, and payment obligations depend on your individual plan, and we verify benefits before care begins where possible.
- Any estimate of coverage or out-of-pocket cost is an estimate, not a guarantee of payment by your plan.
- You remain financially responsible for services rendered in accordance with the financial agreement you sign at the office.
Third-Party Financing and Services
We make third-party patient financing information available as a convenience. Those applications, approvals, interest rates, and account terms are determined solely by the financing company under its own agreement with you.
- We are not a lender, and we do not make credit decisions or set financing terms.
- We do not guarantee approval, any particular rate, or any promotional term.
- Disputes about a financing account are between you and that company.
- Our website also uses third-party features such as maps, review displays, and accessibility tools, which are governed by their own terms and privacy policies.
Intellectual Property
All content on this website, including text, page layouts, graphics, logos, photographs, and the selection and arrangement of that content, is owned by Dallas Spine Center LLC or its licensors and is protected by copyright, trademark, and other laws.
You may view and print pages for your own personal, non-commercial use. You may not copy, republish, distribute, sell, scrape, or create derivative works from this site, or use our name, logo, or content for commercial purposes, without our prior written permission. All rights not expressly granted are reserved.
Acceptable Use
When using this website, you agree not to:
- Use it for any unlawful purpose or in violation of these Terms
- Submit false, misleading, or fraudulent information, or impersonate another person
- Attempt to gain unauthorized access to the site, its servers, or any related systems
- Introduce viruses, malware, or other harmful code
- Use automated tools to scrape, harvest, or overload the site
- Interfere with the site’s operation or with other users’ access to it
- Infringe the intellectual property or privacy rights of others
We may suspend or terminate access to the site, and pursue any available legal remedy, for conduct that violates these Terms.
Reviews, Testimonials, and Submissions
If you submit a review, comment, photo, or other content to us or about us, you grant us a non-exclusive, royalty-free, worldwide license to use, display, and reproduce it in connection with the practice, including on this website and in marketing materials, unless you tell us otherwise in writing.
You are responsible for what you submit, and you agree it is truthful and that you have the right to share it. We may decline to publish, or may remove, any submission at our discretion. We do not offer compensation in exchange for reviews. Please do not include other patients’ information in anything you send us.
Third-Party Links
This site links to websites we do not control. Those links are provided for convenience and do not constitute endorsement or a guarantee of the accuracy or safety of the linked content. We are not responsible for the content, policies, or practices of third-party websites, and accessing them is at your own risk.
Disclaimer of Warranties
This website and its content are provided “as is” and “as available,” without warranties of any kind, express or implied, including implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement.
We do not warrant that the site will be uninterrupted, timely, secure, or error-free, that defects will be corrected, or that the site or its servers are free of harmful components. While we work to keep information current and accurate, medical knowledge, office policies, pricing, insurance participation, and hours change, and content may become out of date.
Limitation of Liability
To the fullest extent permitted by applicable law, Dallas Spine Center LLC and its owners, providers, and staff will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for lost profits, lost data, or business interruption, arising out of or relating to your use of, or inability to use, this website, even if we have been advised of the possibility of such damages.
Our total liability arising out of or relating to this website will not exceed one hundred dollars ($100.00).
This section applies to your use of the website only. Nothing in these Terms limits, waives, or otherwise affects any right or claim you may have arising from professional healthcare services actually rendered to you as a patient, and nothing here excludes liability that cannot lawfully be excluded.
Indemnification
You agree to indemnify and hold harmless Dallas Spine Center LLC and its owners, providers, and staff from any claims, damages, losses, and reasonable attorneys’ fees arising out of your misuse of this website, your violation of these Terms, or your violation of the rights of any third party.
Governing Law and Disputes
These Terms are governed by the laws of the State of Texas and applicable federal law, without regard to conflict of laws rules, except where applicable law requires otherwise. Website-related disputes will be handled by a court with appropriate jurisdiction in Texas, subject to any mandatory jurisdiction or venue requirements.
Talk to us first
If you have a concern, please contact us before filing a claim. Most issues can be addressed by a phone call, and we ask for a reasonable opportunity to work with you on a resolution. This request does not prevent you from exercising your legal rights or meeting a filing deadline.
Time limits
Time limits for bringing claims are determined by applicable law. Nothing in these Terms shortens a limitation period that cannot lawfully be shortened or limits your rights concerning professional healthcare services.
General Provisions
- Changes: We may update these Terms at any time. Changes are effective when posted with a revised date, and your continued use of the site means you accept the updated website terms. Changes to these Terms do not replace your separate SMS consent or authorize additional message categories.
- Severability: If any provision is found unenforceable, the rest of these Terms remain in effect.
- No waiver: Our failure to enforce a provision is not a waiver of our right to enforce it later.
- Entire agreement: These Terms and our Privacy Policy are the entire agreement between you and us regarding this website.
- Accessibility: We work to keep this site usable for everyone. If you encounter a barrier, contact us and we will try to help and to fix it.
